How can I access HCP funding?

In Australia, patients can access Home Care Package (HCP) funding through a process involving the following steps:

Assessment: The first step is to undergo an assessment by the Aged Care Assessment Team (ACAT) or the Aged Care Assessment Service (ACAS). This assessment helps determine the individual’s eligibility for a Home Care Package and the level of support required.

Approval: Once the assessment is complete, and the eligibility is confirmed, the individual will receive an approval letter from the Australian Government. This letter indicates the assigned level of the Home Care Package.

Choose a Provider: With the approval letter in hand, the individual can then choose an approved Home Care Package provider. They have the flexibility to select a provider that aligns with their needs and preferences.

Care Plan Development: The chosen provider will work with the individual to develop a personalized care plan. This plan outlines the specific services and support that will be provided based on the individual’s needs and goals.

Package Activation: After finalising the care plan, the Home Care Package is activated. The approved funding is then allocated to the chosen provider to deliver the agreed-upon services.

Monitoring and Adjustments: The individual’s needs and circumstances may change over time. Regular reviews are conducted to ensure that the care plan remains effective, and adjustments can be made if necessary.

Do you offer home visits for HCP participants?

Yes, we do provide home visits for HCP participants who are unable to come to our clinic for appointments. At the moment, due to our current staff capacity, we can only offer home visits within a 15 km radius from our Pain Free Clinic in Sunnybank Hills. However, we are actively working on expanding the reach of our services in the near future.

Can you help me with my HCP application?

While we are not able to create or manage HCP funding, our team can provide information and assistance to help you understand your plan and how to use your funding effectively to access the services you need.

What happens during an HCP assessment?

During an HCP assessment, our team will evaluate your needs and goals to determine the support and services you require. This assessment helps us develop a tailored plan to address your specific needs and maximize your independence and well-being.
